How to Apply

APPLICATION DEADLINE: DECEMBER 19, 2008 

Applications are accepted all year on a first come first served basis. Please submit your application as early as possible.

How do I know if I’m eligible to attend ChalleNGe?

I am ready to enroll in Project ChalleNGe. What is the next step? 

There are three steps that must be complete before an application can be processed for acceptance. These include the Pre-Application/Application, Medical Application and Mentor Application.

Step 1. Student Application (PDF) - Includes Medical Application Forms. Individual areas of Application can be downloaded here.

Step 2. Medical Application (PDF) - This is the second step of the Cadet Application. Make sure your doctor reads the physical requirements of the program before examining you. Individual areas of Medical Application can be downloaded here.

Step 3. Primary Mentor Application (PDF) - A Student application isn't complete without Primary and Secondary Mentor Application. The mentor will be one of the most important members of your team so make sure you understand what Arizona Project ChalleNGe mentoring is about and what the requirements of a Mentor are.   Choose your nominee(s) wisely!  Having trouble finding a Mentor, check out some resources. Additional Mentor reference sheets can be downloaded here.

If you’re having trouble finding a Mentor, you may want to look into the following:

· Churches
· School and/or Guidance Counselors
· Teachers
· Fire and Police Departments
· YMCA Organizations
· Local Volunteer Groups
· Local Colleges
· Sports Coaches
· Friends of Relatives
· Neighbors 
· Parent’s Co-Workers or Bosses
· American Legion Posts
· Family Friend 

Mentor Requirements
In order to apply to be a Mentor for a Cadet, you must meet the following requirements:

· Same Gender as Cadet
· Over the age of 24
· Non-Family Member
· Cannot live in the same household as the Cadet
· Live in close proximity to the Cadet
· Willing to attend Mentor Training, at Project ChalleNGe
· Committed to maintaining Mentor Relationship during Post-Residental Phase (12 months), which requires completion of monthly report.  

A mandatory drug screen will be conducted on opening day.  Applicants may not enter the Pre-ChalleNGe Phase if they test positive for the presence of illegal drugs.
